POSITION OVERVIEW
The North Simcoe Community Support Navigator (CSN) plays a vital role in connecting individuals, families, and caregivers to appropriate local services that address their health, social, and daily living needs. This key role works with individuals to ensure they have the support they need to remain safely in their home and local community. The CSN will provide timely, accurate, and client-centered referrals to relevant programs and resources.
Reporting to the ILS Director, Community Services, the CSN works closely with local health, social and community support providers to bring the most appropriate local solutions individual care planning. The CSN will work directly with Georgian Bay General Hospital (GBGH) discharge planners and Ontario Health @Home Care Coordinators to facilitate smooth transitions for clients and patients returning to the community or to support their ongoing independence at home. Additionally, the CSN will work with the North Simcoe Ontario Health Team (NS OHT) to identify and implement process improvement initiatives including the development of an efficient and streamlined referral system across North Simcoe.
Location: Midland, on-site, with frequent travel within North Simcoe
Vacancy: 1 full-time (35/hrs/wk.), 12-month contract (Open to considering a secondment agreement)

RESPONSIBILITIES
- Hospital and Ontario Health atHome Collaboration
- Partner closely with discharge planners at Georgian Bay General Hospital (GBGH) to facilitate smooth transitions for patients requiring community support services upon discharge.
- Collaborate with Ontario Health atHome Care Coordinators to align care plans, expedite referrals, and support the ongoing needs of clients transitioning to or requiring community-based care.
- Community Support, Navigation, and Referral Coordination
- Serve as a point of contact for individuals, caregivers, families and service providers seeking information about community support services in North Simcoe.
- Provide tailored guidance by exploring available options, explaining eligibility criteria, and connecting clients to appropriate programs such as personal support, assisted living, housing, food security, transportation, and social supports.
- Facilitate the development of streamlined referral pathways between clients, healthcare providers, social services, and community programs, ensuring timely and effective connections.
- Track, monitor, and document referral activities to guarantee follow-up, resolve barriers to service engagement, and support seamless transitions.
- Service Mapping and Resource Management
- Maintain a detailed and up-to-date inventory of community support services, programs, and providers in the North Simcoe region.
- Explore and support the development of resource guides, databases, and tools to support efficient service navigation.
- Identify service gaps and make recommendations for improvement to address unmet needs within the community.
4. Data Collection and Reporting
- Collect and analyze client interaction data using approved platforms to support decision-making. Prepare actionable reports, including KPIs, client trends, and service gaps. Utilize dashboards to present findings to leadership, funders, and community partners.
5. Stakeholder Collaboration and Engagement
- Actively participate in North Simcoe Ontario Health Teams projects, committees/working groups and community/professional networks
6. Process Improvement and System Development
- Participate in service improvement initiatives to enhance the efficiency of referral pathways and service delivery.
- Identify barriers in the client journey and propose solutions to improve access and outcomes.
QUALIFICATIONS
Education
- Post-secondary degree in Social Work, Health Administration, Community Development or related field required; an equivalent combination of relevant education and directly related experience may be considered.
- Minimum of 3-5 years of experience in a client-focused role within healthcare, social services, or community support settings.
- Demonstrated experience in resource navigation, referral processes, and client care coordination.
Knowledge/Skills
- Knowledge of local/regional community support services in Simcoe County and Ontario Health Central and healthcare systems such as Ontario Health Teams.
- An understanding of principles of person-centered and client-driven approaches to service delivery.
- Demonstrate an understanding and appreciation of cultural differences, including the ability to interact effectively with people from diverse backgrounds and worldviews.
- Familiarity with case management software, referral platforms, and data visualization tools like dashboards.
- Excellent communication skills (verbal, written, and interpersonal) in both French and English are preferred, with the ability to engage effectively with clients, colleagues, and stakeholders and foster inclusive and respectful interactions.
- Demonstrated ability to address and resolve challenges with professionalism and empathy.
- High attention to detail and strong organizational abilities to manage tasks efficiently and meet deadlines.
- Strong customer service, relationship-building, and conflict resolution skills to foster trust and maintain positive interactions.
- Proven ability to handle sensitive information with discretion and maintain a high level of confidentiality.
- This position requires travel within the North Simcoe region. A valid driver’s license, insurance and access to a reliable vehicle required.
- A combination of in-office, some virtual work, and community-based activities will be expected.
- Hours of work: Monday to Friday, 8:30am to 4:30pm
- This position serves as the primary back-up for the after-hours on-call person(s) when needed and to be determined.
